Monitoring aftercare progress and reintegration- Weekly communication with key stake-holders (Clinical team, referent, family members)
- Coordination of educational development support and/or career coaching
- Coordination of local clinical care which may include: intensive outpatient programming, therapy, psychiatry, etc.
- Fees for external clinical services are not included in the fees for Post Precision Recovery, and must be paid for separately by the client.
- Coordination of wellness services which may include: acupuncture, physical training, nutritionist, etc.
- Fees for external wellness services are not included in the fees for Post Precision Recovery and must be paid for separately by the client.
- Bi-monthly family meetings (phone or in person, when feasible) to provide coaching to further help support recovery efforts
- Assistance with building a weekly schedule that addresses key life areas and supports the maintenance phase of recovery
- Short and long-term goal setting
- In the case of relapse, re-evaluation and connection to a higher level of care, at the recommendations of John Post to client and family.
